105例小儿反复肺炎回顾性分析

Retrospective Analysis of 105 Cases of Children with Recurrent Pneumonia

目的:探讨小儿反复肺炎的基础疾病和诊断方法。方法:回顾性分析了2005年1月～2009年12月105例反复肺炎患儿的临床资料。结果:105例反复肺炎患儿中,98例(93%)存在基础疾病,其中伴先天性或获得性呼吸系统疾患60例(57%),是造成小儿反复性肺炎最常见的原因,其次是先天性心血管疾病26例(24%)。反复肺炎患儿各种基础疾病各有其病史、体格检查和X线胸片表现特点,在此基础上多数基础疾病通过多项检查确诊。结论:儿童反复肺炎多存在基础疾病,病史、体格检查和X线胸片可以为进一步的检查提供重要线索,特殊检查如胸部CT和纤维支气管镜等是多数小儿反复肺炎基础疾病诊断的主要依据。

Objective：To explore the underlying illnesses and diagnostic methods of recurrent pneumonia in children.Methods：The clinical data of 105 children with recurrent pneumonia from January 2005 to December 2009 were analyzed retrospectively.Results：Underlying illnesses were identified in 98 children （93%）,including congenital or acquaired anomalies of respiratory system 60（57%）,congenital heart 26 例（24%）,Each of the underlying illnesses had its own characteristics of history,physical examination and chest radiograph.Based on these characteristics,most underlying illnesses were diagnosed by multinomial investigations.Conclusion：Most children with recurrent pneumonia have an underlying disease.History ,physical examination and chest radiograph can provide important clues for the investigation.Special investigations,such as bronchoscopy and chest CT play a major role in the diagnosis.
