阴式肌瘤剔除术治疗子宫肌瘤的临床研究

Vaginal Myomectomy for Uterine Fibroid:A Clinical Study

目的比较阴式肌瘤剔除术和经腹肌瘤剔除术治疗子宫肌瘤的临床疗效。方法将来我科治疗的92例子宫肌瘤患者随机分为两组,其中观察组46例,行阴式剔除术切除子宫肌瘤;对照组46例,行经腹肌瘤剔除术切除子宫肌瘤,比较两种手术方式的临床疗效。结果观察组术中失血量为(69.3±18.7)mL,手术时间为(72.4±13.5)min,肠功能恢复时间为(12.7±3.9)d,肛门排气时间为(15.4±4.8)h,住院天数为(6.7±1.3)d;对照组术中失血量为(99.3±23.7)mL,手术时间为(90.9±16.6)min,肠功能恢复时间为(16.5±5.1)d,肛门排气时间为(20.2±5.9)h,住院天数为(8.8±2.7)d,观察组的疗效明显优于对照组,差异有统计学意义(P<0.05);观察组复发率为6.5%,对照组复发率为10.9%,两组差异无统计学意义(P>0.05)。结论与经腹肌瘤剔除术相比,阴式肌瘤剔除术治疗子宫肌瘤效果显著,具有手术时间短和术中出血少等优点,值得临床推广应用。

Objective To compare the clinical effect of vaginal myomectomy and abdominal myomectomy on uterine fibroid.Methods Eighty-two cases of uterine fibroid in our department were divided into two groups,with 46 in each： observation group and control group.The observation group was treated by vaginal myomectomy and the control group was treated by abdominal myomectomy.The clinical efficacy was compared.Results The blood loss,operative time,intestinal function recovery time,anal exhaust time and hospital stay of the observation group were（69.3±18.7）mL,（72.4±13.5）min,（12.7±3.9）d,（15.4±4.8）h and（6.7±1.3）d,and those of the control group were（99.3±23.7）mL,（90.9±16.6）min,（16.5±5.1）d,（20.2±5.9）h and（8.8±2.7）d.The clinical effect of the observation group was significantly better than that of the control group,with a significant difference（P0.05）,and the recurrence rate of the observation group was 6.5% and that of the control group 10.9%,with no significant difference（P0.05）.Conclusion Compared with abdominal myomectomy,the effect of vaginal myomectomy in the treatment of uterine fibroids is remarkable.It can shorten the operative time and reduce bleeding,worthy of being popularized.
