磁共振三维导航血管成像技术在诊断脑动脉瘤中的应用 被引量:1

摘要 目的 :本文利用最新的磁共振三维导航血管成像技术 (MR 3DNavigatorVascularImaging)诊断和观察脑动脉瘤 ,重点在于显示动脉瘤的颈部和开口 ,为动脉瘤的治疗提供依据。方法 :经手术及血管造影证实脑动脉瘤 3 5例共 40个 ,所有病例都进行脑组织磁共振成像和磁共振血管成像 ,将血管成像扫描包括 3DTOF或 3DTOF伴有ZIP技术的原始图像在工作站进行三维导航的成像处理 ,包括外视镜式血管表面重建 (Exoscopicsurfacerendering)和仿真内窥镜式血管表面重建 (Virtualintravascularendoscopy ,VIE) ,并与其它血管后处理技术进行比较。结果 :MR 3DNavigator对动脉瘤颈的显示率为 90 % ,对脑小动脉瘤的检出率为 94% ,显示动脉瘤的形态 ,确切部位以及与临近血管的关系明显好于常规技术和原始图像 ,VIE可显示血管和动脉瘤的腔内结构如血栓和动脉瘤的开口等 ,并且图像逼真 ,直观和能进行飞航观察。结论 :MR 3DNavigator在诊断和观察动脉瘤的颈部和开口 ,形态大小和确切部位及与邻近血管的关系明显优于常规后处理技术 ,并对动脉瘤手术或介入治疗方案的制订提供了依据 。 Objective:To assess MR 3D Navigator Vascular Imaging for the detection and characterization of the intracranial aneurysms with focus on its neck and opening for planing the treatment of aneurysms.Methods:A total of 35 patients with 40 aneurysms surgically or DSA proven were studied.MR 3D Navigator Vascular Imaging were performed using source images from 3D TOF or 3D TOF with ZIP (Zerofill Interpolation Processing).MR 3D Navigator included exoscopic surface rendering and Virtual intravascular endoscopy (VIE).Results:MR 3D Navigator provided better visualization of the neck of the aneurysm (90%),its location,morphology and adjunct vessels,and better detection of small aneurysms (94%) than original source images and other postprocessing techniques.VIE clearly showed the inner surface of the vessels and aneurysms including the orifice and thrombosis etc..Conclusion:MR 3D Navigator can better detect the presence,location,morphology and neck of the aneurysm and help for making plan for surgical and interventional treatment and follow up the size,patency of the aneurysm after intravascular embolization and other therapy.
