重型颅脑伤中外侧裂区脑挫裂伤43例手术及诊治分析

Experience in operative and clinical treatment of 43 cases with severe craniocerebral injury with cerebral contusion located in lateral fissure area

目的探讨颅脑损伤中外侧裂区损伤的手术及治疗方法。方法回顾性分析笔者所在医院2005年6月～2010年6月的43例外侧裂区脑挫裂伤的重型颅脑外伤患者的临床表现、手术及治疗情况与预后的关系。结果 43例手术患者,按GOS评价:恢复良好6例,中重残26例,植物生存3例,死亡8例。结论颅脑损伤合并侧裂区损伤,及时手术、充分减压及术中保护好侧裂血管、术后综合治疗,是有效的临床治疗措施。

Objective To study the surgical techniques and clinical treatment of severe craniocerebral injury with cerebral contusion located in lateral fissure area. Methods From June 2005 to June 2010, 43 patients of cerebral contusion located in lateral fissure area underwent the surgical procedure, which clinical manifestations, therapeutic effects and prognosis was analyzed retrospectively. Results All patients revelation were operated, and 1 months after operation, which prognosis was assessed by GOS, and showed as follows ：6 were recovered well,26 moderately or severely disabled ,3 persistent vegetative states, and 8 died. Conclusion This decompressive operative methods with standard large craniotomy and internal decompression was the most effective, meanwhile which protection the blood vessel located in lateral fissure area and postoperative comprehensive treatment had the same clinical significance.
