摘要
目的 探讨改良的后侧入路治疗肩胛骨骨折的手术疗效.方法 采用改良的后侧入路治疗肩胛骨骨折患者18例.骨折根据Hardegger等分型方法,肩胛冈骨折5处,肩胛体骨折15处,肩胛颈骨折11处,盂缘骨折3处,盂窝骨折4处,通过改良的后侧入路,对不同类型的肩胛骨骨折均采用重建钢板,螺丝钉内固定.结果 18例患者均获得随访,平均随访时间18个月（8～32个月）.根据Rowe疗效评价标准,优12例,良3例,可2例,差1例,优良率为83.3%.结论 改良的后侧入路操作简单,暴露充分,可以治疗大部分肩胛骨骨折,是一种安全有效的方法.
Objective To investigate the effect of operative treatment of scapular fracture through modified posterior approach. Methods From January 2002 to June 2009,18 patients with scapular fractures were treated by open reduction and internal fixation through modified posterior approach. According to Hardedge classification system, there were 5 cases of scapular spine fractures, 15 cases of scapular body fractures,ll cases of scapular neck fractures,3 cases of glenoid rim fractures and 4 cases of glenoid fossa fractures. Results All of the patients were followed up for 18 months on average（range：8 to 32 months）. According to Rowe scores system, the efficacy was excellent in 12 patients,good in 3 patients, fair in 2 patients and poor in 1 patient. 83.3% of the patients had excellent or good outcomes. Conclusion The modified posterior approach has the advantages of easy fixation and wide exploration and is effective for most types of scapular fractures.
