摘要
目的:探讨联合检测血清和胸水中四种肿瘤标记物对肺癌的诊断意义。方法:采用化学发光和电发光法对60例肺非癌病人和52例肺癌病人血清和胸水的癌胚抗原(CEA)、糖类抗原125(CA125)、细胞角蛋白片段19(CYFRA21-1)和神经原特异性烯醇化酶(NSE)水平进行检测。结果:肺癌患者胸水中CEA为237.4±209.1 ng/ml,CA125为234.0±154.1 U/ml,NSE为38.4±26.5 ng/ml,CYFRA21-1为64.7±61.8 ng/ml,显著高于本组血清(P<0.01)和非肺癌疾病组的血清和胸水中的水平(P<0.01)。胸水中四项联检对肺癌的敏感性达96.6%,而对应的血清中四项联检对肺癌的敏感性则为92.3%。结论:联合检测血清和胸水中CEA、CA125、NSE和CYFRA21-1对肺癌的诊断具有重要意义。
Objective:To study the diagnostic value of detection of CEA,CA125,CYFRA21-1 and NSE in pleural fluid samples for pleural metastasis of lung cancer.Methods: Immunoprotein quantity of CEA,CA125,CYFRA21-1 and NSE was analyzed in pleural fluid and serum from patients with lung cancer(52 cases) and in pleural fluid from non cancerous patients(60cases) by using Chemiluminescence immunoassay and Electrochemiluminescence immunoassay.Results:The levels of CEA,CA125,CYFRA21-1 and NSE in pleural fluid of patients with lung cancer were significantly high than those of non cancerous patients(P0.01).The levels of CEA,CA125 CYFRA21-1 and NSE in pleural fluid were obviously higher than those in serum(P0.01).The sensitivity of detection CEA+CA125+CYFRA21-1+NSE in pleural fluid and serum were 96.6% and 92.3% respectively.Conclusion:Combined detection of CEA,CA125 CYFRA21-1 and NSE in pleural fluid and serum might be helpful for diagnosing pleural metastasis of lung cancer
