64排螺旋CT门静脉血管成像的临床应用

The reasearch about the 64-Slice spiral CT portography and its clinical use

目的探讨64排螺旋CT门静脉血管成像的临床应用价值。方法 120例疑肝脏疾病者进行门静脉血管成像检查,其中表现正常者20例作为对照,肝硬化并门静脉高压80例(42例并肝癌),其他20例,双筒高压注射对比剂的同时行64排螺旋CT容积扫描,利用计算机后处理功能得到门静脉立体图像。结果①门静脉血管成像图像好坏与扫描延迟时间合理选择和扫描参数设计密切相关;②能清楚显示1～5级门静脉血管结构;③精确定位肝内病灶;④门静脉造影可检测门静脉高压侧支循环的范围和估计严重程度,以及显示多种疾病。结论门静脉血管成像是门静脉无创性检查的可靠的方法,具有一定的临床应用价值。

Objective To evaluate technique and clinical value of SCTP by 64-slice spiral CT.Methods One hundred and twenty cases who were suspected of hepatic diseases underwent SCAP,20 cases were normal,80 cases had liver cirrhosis and portal vein high pressure（42 cases of them had liver cancer）,other 20 cases.After bolus injection contrast agent in high speed,64-Slice Spiral CT volume scaning underwent at once.The portal vain 3D images were obtained with the computed functions.Results ①The images quality was correlated to the scanning delay time and sanning parameter design.②1~5 grade structure of portal vein could be delineated clearly.③The locations of hepatic diseases were very precise.④It could inspect the range of hepatic portal vein high pressure about siding circulation and estimate its pathological changes and so on.Conclusion SCTP is reliable and has no traumatic method exeming portal vein.The methoel has certain clinical application values.
