摘要
目的观察3种短疗程方法治疗重度外阴阴道假丝酵母菌病(SVVC)患者的疗效。方法将148例SVVC患者随机分为3组。A组(n=50):克霉唑栓500 mg,阴道用药,每晚1次,第1、4日。B组(n=48):氟康唑150 mg,口服,1次/d,第1、3日;C组(n=50):单次口服氟康唑150 mg,同时应用克霉唑栓500 mg,阴道用药1次。观察3组疗效。停药7~14天为近期疗效,停药30天为远期疗效。结果 A组近期及远期临床有效率分别为78.00%、76.00%,真菌转阴率分别为88.00%、88.00%;B组近期及远期临床有效率分别为81.25%、81.25%,真菌转阴率分别为85.42%、87.50%;C组近期及远期临床有效率分别为80.00%、78.00%,真菌转阴率分别为92.00%、90.00%。3组近期及远期临床有效率和真菌转阴率差异均无统计学意义(P〉0.05)。结论 3种方法治疗SVVC效果相当;局部用药患者临床症状消失快;短疗程用药,患者依从性好;都是治疗SVVC的有效措施。
Objective To evaluate the therapeutic effects of the 3 short-term treatment regimens for severe vulvovaginal candidiasis(SVVC).Methods 148 patients with SVVC were randomized into 3 groups.Group A(n=50): clotrimazole 500 mg administered via vagina,once per night,for the first and the fourth days;Group B: fluconazole 150 mg administered orally,once daily,for the first day and the third days;Group C: single dose of fluconazole 150 mg administered orally and single dose of clotrimazole 500 mg administered via vagina.The therapeutic effects of the 3 regiments were observed.The short-term effect was designated to 7–14 days after the treatment and the long-term effect was set to 30 days after the treatment.Results The efficacy rate in group A was 78.00% and 76.00% for short-term and long-term effects,with a mycological negative conversion rate of 88.00% and 88.00%,respectively;The efficacy rate in Group B was 81.25% and 81.25% for short-term and long-term effects,with a mycological negative conversion rate of 85.42% and 87.50%,respectively.The efficacy rate was 80.00% and 78.00% for short-term and long-term effects,with a mycological negative conversion rate of 92.00% and 90.00%,respectively.There was no significant difference among the 3 regiments as for the short-term and long-term therapeutic effects and the negative conversion rates of the fungi(P 0.05).Conclusion All the 3 regiments for SVVC have satisfactory effects owing to the timely elimination of clinical symptoms by local medication and coordination from patients.
