40 Hz Multiple Auditory Steady-state Responses in Adults with Normal Hearing

摘要 目的研究正常听力成人40Hz多频听性稳态反应(40Hz多频ASSR)的基本特性,探讨其可能的临床应用价值。方法对15例正常听力成人(男4人,女11人,年龄19~37岁,平均22.5±4.1岁,双耳各频率气导纯音听阈均在20dBHL以内)进行40Hz多频ASSR测试,刺激信号是载波频率为0.5、1、2和4kHz的调幅调制音,调制频率左耳为32、40、56、64Hz,右耳为34、42、58、66Hz,双耳由ER-3A插入式耳机8个频率同时给声。应用方差分析法对各频率的阈值、40dBHL刺激强度下的反应幅值进行统计学分析。结果 40Hz多频ASSR在0.5、1、2、4kHz的阈值分别为39.23±10.72、35.67±8.17、36.09±8.39和38.26±11.54dBHL,各频率间差异无统计学意义(P>0.05);在40dBHL刺激强度下0.5和1kHz的幅值显著高于2和4kHz。结论 40Hz多频ASSR的阈值各频率之间没有显著性差异,在相同刺激强度下其反应幅值低频高于高频;40Hz多频ASSR对听阈的评估有一定的参考价值。 Objective To investigate the basic characteristics of multiple 40 Hz auditory steady--state responses (ASSRs) in adults with normal hearing, and to discuss its clinical applications. Methods 15 adults were recruited in this study and their pure tone thresholds from 0.25-8 kHz were within 20 dB HL. The subjects were 11 females and 4 males. Air--conducted stimuli were present to both ears through ER--3A insert earphones. Stimuli for 40 Hz multiple ASSR consisted of sine waves at carrier frequencies of 0.5, 1, 2 and 4 kHz, 90% amplitude--modulated at frequencies of 32, 40, 56 and 64 Hz for the left ear and 34, 4Z, 58 and 66 Hz for the right ear, respectively. The data were processed by 2--way repeated measures analysis of variance (ANOVA). Results ①The mean and standard deviations of multiple 40 Hz ASSR thresholds at 0.5, 1, 2 and 4 kHz were 39.23±10.72, 35. 67±8. 17, 36.09±8.39 and 38.26±11.54 dB HL, respectively. An analysis of thresholds indicated no significant differences among frequencies. ② Significant amplitude differences were noted among carrier frequencies with the biggest at 0.5 and 1 kHz. Conclusion The thresholds of multiple 40 Hz ASSR at 0.5, 1, 2 and 4 kHz do not differ significantly from each other; the amplitudes at frequencies of 0.5 and 1 kHz are significantly larger than those at 2 and 4 kHz; the multiple 40 Hz ASSR can provide information to thresholds evaluation.
