促红细胞生成素联合甲泼尼龙治疗急性药物性间质性肾炎的疗效研究 被引量:10

摘要 目的探讨促红细胞生成素(EPO)联合甲泼尼龙冲击治疗急性药物性间质性肾炎(AIN)的临床疗效。方法 32例AIN患者被随机分为治疗组和对照组,治疗组16例患者采用EPO联合甲泼尼龙治疗,对照组16例患者单用甲泼尼龙冲击治疗。观察两组患者的少尿期、多尿期、血尿和蛋白尿消失时间及肾功能恢复正常时间,比较两组患者的疗效。结果治疗组患者的少尿期、多尿期、血尿和蛋白尿消失时间及肾功能恢复正常时间均短于对照组,差异有统计学意义(P<0.01)。治疗组16例患者中,完全缓解15例,部分缓解1例;对照组16例患者中,完全缓解10例,部分缓解6例,两组患者的疗效间差异有统计学意义(P=0.036)。结论 EPO联合甲泼尼龙冲击治疗AIN比单用甲泼尼龙治疗效果更佳。 Objective To investigate the clinical efficacy of erythropoietin(EPO) combined with methylprednisolone in treating drug-induced acute interstitial nephritis(D-AIN).Methods 32 D-AIN patients were randomly divided into treatment group(n=16) and control group(n=16).The treatment group was given erythropoietin in combination with methylprednisolone while the control group was given methylprednisolone alone.The results of urine routine tests,convalescence time of renal function,therapeutic efficacy and prognosis were observed.Results The period of oliguria and polyufia,the duration of hematuria and proteinuria and convalescent time of renal function in treatment group were all significantly shorter than that of the control group(P〈0.01).15 cases showed complete remission and 1 case showed partial remission in treatment group;10 cases showed complete remission and 6 cases showed partial remission in control group.The difference between the two groups was significant(P=0.036).Conclusion The efficacy of erythropoietin combined with methylprednisolone is better than methylprednisolone alone in treating D-AIN.
