2005-2009年四川省大骨节病病情调查结果分析

Investigation Results of Kaschin-Beck Disease in Sichuan Province,2005-2009

目的了解四川省大骨节病病情现状,指导重点病区的预防控制工作,为评估控制和消除大骨节病提供科学依据。方法各病区县以村为单位,对调查点内7～12岁儿童进行大骨节病临床检查和右手正位X线拍片。结果 2005-2009年共调查7～12岁儿童13472人,临床检出阳性72人,平均临床检出率0.53%(0～1.07%);X线检出阳性373人,平均X线检出率2.77%(1.55%～4.86%),干骺端阳性率为2.06%(0.47%～3.68%),骨端阳性率为0.70%(0.28%～4.39%),骨骺阳性率为0.05%(0～0.09%),腕骨阳性率为0.01%(0～0.09%),仅有1例三联征。X线检出率大于10%的病区村所占比例呈下降趋势,活跃病区村主要集中在阿坝州地区。结论四川省大骨节病病情稳定,儿童新发得到有效控制,仅个别地区病情仍然较重。提示今后应该加强重点地区监测,在病情活跃地区采取有针对性的防治措施。

Objective To investigate the status of Kaschin-Beck disease（KBD） in Sichuan province,guide prevention and control work in focus areas and provide a scientific basis for assesement of the control and elimination of Kaschin-Beck disease.Methods A village as a unit,did clinical detection and shoot right hand X-ray for 7-12 year-old children.Results 2005-2009,a total of 13 472 children aged 7-12 were clinical detected,clinical detection of positive 72,the average positive rate was 0.53% （0-1.07%）; X-ray detection of positive 373,the average positive rate was 2.77% （1.55%-4.86 %）,the positive rate of metaphysis was2.06%（0.47%-3.68%）,the positive rate of bone side was 0.70%（0.28%-4.39%）,the positive rate of epiphysis was 0.05%（0-0.09%）,the positive rate of wrist was 0.01%（0-0.09%）,only one case was triad. The proportion of X-positive rate of greater than 10% of the endemic villages decreased,active endemic villages concentrated in the Aba area.Conclusions Kashin-Beck disease in Sichuan Province is in stable condition,the emergence of Kaschin-Beck disease of the children has been controlled effectively,only individual regions condition is still serious.We should strengthen monitor in key areas and adopt appropriate prevention and control measures in KBD active regions in furture.
