早期心理干预对冠脉造影患者负性情绪的影响

Effect of Early Psychological Intervention on the Negative Sentiment in Patients Undergoing Coronary Angiography

【目的】探讨早期心理干预对冠脉造影患者负性情绪的影响。【方法】对148例冠心病拟行冠状动脉造影患者随机分为对照组和干预组各74例。对照组采取常规护理；干预组在常规护理的基础上，进行心理干预。采用焦虑自评量表（SAS）、抑郁自评量表（SDs）和症状自评量表（SCL-90）分别于预约时和术后1周对两组患者进行评分。【结果】两组患者在预约时SDs、SAS评分比较差异无统计学差异（P〉0．05），术后1周干预组SDS、sAS评分显著低于对照组（P〈0．01），术后1周SCL-90量表中干预组躯体化、强迫症状、人际关系、抑郁、焦虑、敌对、恐怖和偏执分值均下降，与对照组相比有显著差异（P〈0.01或P〈0．05）。【结论】早期心理干预能改善冠脉造影患者的心理状态，提高生活质量。

[Objective] To explore the effect of early psychological intervention on the negative sentiment in patients undergoing coronary angiograpby. [Methods] A total of 148 patients with coronary heart disease undergoing coronary angiography were randomly divided into control group and intervention group with 74 in each. The control group was treated with routine care, while the intervention group was treated with psychological intervention and health education on the basis of routine care. The self-rating anxiety scale（SAS）, selfrating depression seale（SDS） and symptom checklist-90 （SCL-90） were carried out for the patients in two groups when the appointment was received and 7 days after operation. [Results] There were no significant differences between two groups in the scores of SDS and SAS when the appointment was received （ P〉0.05）, while the scores of SDS and SAS in the intervention group were significantly lower than those in the control group 7 days after operation（ P 〈0.01）. The scores of SCL- 90 scale 7 days after operation in the intervention group declined in somatization, obsessive compulsive disorder, interpersonal relationship, depression, anxiety, hostility, fear and crankiness, and there was significant difference between the intervention group and the control group（ P 〈0.01 or P 〈0.05）. [Conclusion] Early psychological intervention can improve the psychological status and the quality of life among patients undergoing coronary angiography.
