非翻瓣术式与翻瓣术式在义齿种植中的临床效果比较

Comparative Study Between Flapless and Flap Operations for Oral Implant

【目的】探讨环切非翻瓣种植术式与常规翻瓣术式的优缺点，为临床种植义齿的手术方式提供更好的选择。【方法】通过对787颗非埋植种植义齿（其中ITI431颗，OSSTEM-SS356颗）的研究，评估非翻瓣术式（n=522）与翻瓣术式（n=265）患者的术后自觉症状，术后出血情况，术后7d牙龈状况，及术后3个月种植体周牙槽骨在状况。【结果】环切非翻瓣术式在患者自觉症状，牙龈红肿状况，3个月后种植体边缘骨状况均优于传统翻瓣种植术式。【结论】在牙槽骨宽度足够的状况下，采用环切非翻瓣术式，能有效减轻患者的术后反应，防止种植体边缘骨的吸收。

[Objective] To evaluate of the advantage and disadvantage of circumcise operation with flap and without flap for oral implant in order to find the better method of implant surgery. [Methods] The total of 787 non-embedding implant dentures including 431 ITI and 356 OSSTEM-SS were studied. The patients postoperative pain, subjective symptom, bleeding, gingival condition in 7 days after operation and the state of alveolar bone around the implant in 3 months after operation were evaluated. The data were analyzed with statistical software SPSSll. 5. [Results]The subjective symptom, gingival flare and the state of alveolar bone around the implant in 3 months after surgery by circumcise operation without flap were superior to those by routine flap implant operation . [Conclusion] Circumcise operation without flap under enough alveolar bone width can ef fectively alleviate the postoperative reaction of the patients and prevent the absorption of alveolar bone around the implant.
