慢性淋巴细胞性甲状腺炎合并甲状腺癌17例诊治分析

目的 总结慢性淋巴细胞性甲状腺炎合并甲状腺癌的临床特点、诊断和治疗方式.方法 回顾性分析1994年1月至2009年1月收治的17例慢性淋巴细胞性甲状腺炎合并甲状腺癌的诊治资料,结合有关文献进行综合分析.结果 17例患者均行手术治疗,随访13例未见复发和死亡,随访时间最长达8年.慢性淋巴细胞性甲状腺炎和甲状腺癌两者呈现的发病率差异无统计学意义（P〉0.05）.结论 慢性淋巴细胞性甲状腺炎合并甲状腺癌发病率低,术前诊断困难,提高对此病的认识及掌握手术探查指征非常重要.慢性淋巴细胞性甲状腺炎合并甲状腺癌患者的复发率低,预后相对较好.

Objective To clarify the clinical characteristics, diagnosis and treatment of chronic lymphocytic thyroiditis（CLT） concomitant with thyroid carcinoma（TC）. Methods The clinical data of 17 CLT patients accompanied with TC who were admitted to Beijing Tongren Hospital from 1994 to 2009 were retrospectively analyzed and the related literatures were reviewed. Results All the 17 patients underwent surgical treatment and 13 patients had no recurrence and death during the follow - up period high up to 8 years. There was no statistically difference in the morbidity between patients with CLT and TC （ P 〉 0.05 ）. Conclusion Clinically, CLT coexisting with TC is a rare thyroid disease. CLT is considered to be a sort of preneoplastic condition. It is very important to increase the recognition of CLT coexisting with TC because of its low morbidity and diagnosis being difficult before the operation. CLT concomitant with TC rarely reoccurs and has good prognosis.
