恶性肿瘤患者经外周静脉置入中心静脉导管后血栓形成的治疗

Treatment of vein thrombosis induced by peripherally inserted central venous catheter in patients with malignant tumors

目的探讨恶性肿瘤患者PICC后血栓形成的治疗。方法回顾性分析了6例恶性肿瘤患者PICC后血栓形成的治疗方法及效果。结果 1例拔管困难患者经PICC管内注射尿激酶3日后顺利拔管,2例滤器置入后留置导管进行溶栓的患者5日后均顺利融通并取出滤器,其余3例未行溶栓的患者经抗凝等治疗后症状均得以明显缓解。结论恶性肿瘤患者PICC后血栓形成进行抗凝治疗及必要时溶栓治疗疗效确切。

Objective To study the treatment of thrombosis associated with peripherally inserted central venous catheter（PICC） in patients with malignancy.Methods Treatment of thrombosis associated with PICC in 6 patients with malignancy were retrospective analyzed.Results One patient of difficult extubation accepted urokinase injection via the PICC tube,and the tube was extubated easily after 3 days.Two patients which accepted catheter-directed thrombolysis after filter placement were successful thrombolysis and the filters were removed 5 days later.The others accepted anticoagulation therapy and their symptoms were signif icantly alleviated.Conclusion The therapy of anticoagulation and thrombolysis when necessary has a certain effect to the thrombosis associated PICC in patients with malignancy.
