串联皮瓣修复下肢大面积软组织缺失

Repairing soft tissue defect of lower limbs by dorsal foot-scapular combined skin flap

目的探讨足背动脉皮瓣串联肩胛皮瓣治疗下肢大面积皮肤软组织缺损的治疗效果。方法以显微外科技术为手段,运用足背动脉皮瓣串联肩胛皮瓣治疗下肢皮肤软组织缺损10例。结果10例皮瓣8例全部一期成活,皮瓣质地柔软,形态及功能满意,供区无明显疤痕遗留;2例术后2d换药时皮瓣远端周缘部分静脉回流障碍,经拆除部分缝线后换药一期愈合。结论足背动脉皮瓣串联肩胛皮瓣血供可靠,不牺牲主干血管,对供区外观及功能无不良影响,蒂长设计转移灵活,是修复下肢大面积软组织缺损的理想供区。

Objective Investigate the therapeudic effect of dorsal foot-scapular combined skin flap on soft tissue defect of lower limbs.Methods Ten cases with soft tissue defect of lower limbs were repaired with reversed foot-scapular combined skin flap.In these cases,7 patients had soft tissue defect of calf-knee-thigh after acute wound.Two had soft tissue defect of all the calf.One had scars of large area in lower limb after burn.Results All flaps wered surrived in 10 patients.The function and shape of recipient and donor site were satisfactory.Conclusions Dorsal foot-scapular combined skin flap has reliable blood supply,is long-pedicled,simple,and dose not sacrifice major vessel,thus is a ideal donor site for soft tissue defect repairing of lower limbs.
