双源CT双能量技术对泌尿系结石成分的研究 被引量:13

摘要 目的探讨双源CT对泌尿系结石成分的定性诊断价值;比较单能扫描与双能扫描的辐射剂量。方法收集因腹痛临床怀疑泌尿结石的病例995例,其中男性571例,女性424例,分别行双源CT单能量和双能量扫描,单能量扫描90例,双能量扫描94例,扫描后的数据传至工作站,装入双能量评估软件进行分析并分别统计两种能量图像中结石的CT值差值、比值,对随访得到的体外结石采用标准结石定性分析化学试剂进行分析;不同成分的CT值差值、比值比较,比较两种能量扫描的辐射剂量,采用SPSS11.5软件包进行独立样本t检验,P<0.05认为差异具有统计学意义。结果单能和双能两种模式的辐射剂量比较不具有统计学意义(P>0.05)。尿酸盐与非尿酸结石在两种能量状态下差值、比值比较均具有统计学意义(t=15.043,P<0.05;t=2.398,P<0.05)。结论双源CT是诊断泌尿系结石的一种新方法,它不仅确定结石的大小、位置和泌尿系存在的梗阻,而且还可以对结石成分定性分析。 【Objective】 To explore the qualitative diagnosis of dual-source CT on urinary urate stones and non-urate stones and analyze the radiation dose of single energy and dual energy.【Methods】 collection of clinical suspicion for abdominal pain cases of 995 cases of urinary stones,of which 571 cases of male,female 424 cases,were performed dual-source CT single energy and dual energy scanning,single-energy scan in 90 cases,94 cases of dual energy scanning,the data transmitted to the workstation,mount the dual energy assessment software to analyze and tatistics were two stones energy CT value images ifference,the ratio of follow-up obtained in vitro using standard stone stone qualitative analysis of chemical reagents.Different components of the CT value of the difference,ratio and the two energy scan radiation dose,using independent sample t SPSS11.5 package test,P0.05 considered statistically ifference.【Results】 Single energy and dual energy comparison of two models of the radiation dose was not statistically difference(P0.05).Two types of stone in the difference between two energy states,the ratios were compared statistically significant(t=15.043,P0.05;t=2.398,P0.05).【Conclusion】 Dual source CT diagnose urinary stones,which not only determine the stone size,location,presence of urinary obstruction,but also qualitatively analyze of stone composition.
