甲状腺癌颈清扫术后严重乳糜漏的综合治疗(附5例报告)

Combined treatment of severe chylous leakage after operation of thyroid carcinoma(A reports of 5 case)

目的观察强负压吸引结合肠外营养支持在甲状腺癌术后乳糜漏治疗中的作用。方法对5例甲状腺癌术后严重乳糜漏的病人,使用强负压吸引与肠外营养支持,记录治疗前、后颈部引流量及创口愈合情况。结果经强负压吸引与肠外营养支持及治疗,颈部淋巴液引流量迅速减少,4例经保守治疗成功,一例经再次手术治愈。结论通过强负压吸引与肠外营养支持与可有效地减少甲状腺癌病人术后颈部乳糜漏引流量,促进瘘口的愈合。

Objective To observe the role of high negative pressure drainage combined with parenteral nutrition in treatment of chylous leakage after thyroid carcinoma operation.Methods High negative pressure drainage combined with parenteral nutrition were used in treatment of 5 cases of chylous leakage after thyroid carcinoma operation.The fluid volume from neck drainage and condition of the wound healing were recored.Results Fluid volume rapidly reduced after these treatments.4 cases were cured after conservative treatments and 1 case was cured after reoperation.Conclusion Using high negative pressure drainage combined with parenteral nutrition would obviously reduce fluid volume from neck drainage after thyroid carcinoma operation and promote fistulous healing.
