血清肿瘤标志物在消化系统恶性肿瘤诊断中的价值

The Clinical Value of Blood Serum Tumor Markers in Malignant Neoplasms of Digestive System

目的探讨血清肿瘤标志物在消化系统恶性肿瘤诊断中的价值。方法 692例消化系统疾病患者应用化学发光法检测血清癌胚抗原(CEA)、甲胎蛋白(AFP)、糖类抗原125(CA125)、糖类抗原199(CA199)、糖类抗原153(CA153)的水平。结果消化系统恶性肿瘤患者血清CEA、CA199、CA125水平均高于消化系统良性疾病患者,差异均有统计学意义(P<0.05或P<0.01);肝癌,胃、食管癌和胰腺癌患者血清CA153水平高于良性消化系统疾病患者,差异均有统计学意义(P<0.05);肝癌患者血清AFP水平明显高于其他疾病患者,差异均有统计学意义(P<0.01)。肝癌患者血清AFP阳性率为89.5%;胃、食管癌,结直肠癌患者血清CEA阳性率分别为53.2%、52.4%,胰腺癌患者血清CA199阳性率为93.1%。结论血清CEA、AFP、CA153、CA125、CA199可作为消化系统恶性肿瘤的诊断指标,联合检测可提高诊断的敏感性。

Objective To investigate the clinical value of blood serum tumor markers in malignant neoplasms of digestive system.Methods The AFP,CA125,CA199 and CA153 in blood serum of 692 patients digestive system diseases were detected with chemiluminescence.Results The CA125,CA199 and CA153 of the patients with malignant neoplasms of digestive system were higher than those of the patients with benign diseases of digestive system（P0.05,P0.01）;the CA153 of liver cancer,stomach cancer and pancreatic cancer were higher than those of the patients with benign diseases of digestive system（P0.05）;the AFP of liver cancer were higher than those of the patients with benign diseases of digestive system or other cancers（P0.01）.The positive rate of AFP in the patients with liver cancer was 89.5%;the positive rates of CEA in the patients with the stomach cancer and esophageal cancer,and the colorectal cancer were 53.2% and 52.4%;the positive rate of CA199 in the patients with pancreatic cancer was 93.1%.Conclusions CEA,AFP,CA153,CA125 and CA199 can be used as indicators in malignant neoplasms of digestive system,joint detection can increase the sensitivity.
