颈动脉斑块稳定性与急性脑梗死的炎性机制分析

Analysis of inflammation and destabilization of carotid plaque with acute ischemic stroke

目的探讨炎症因素与急性脑梗死患者颈动脉斑块稳定性的关系。方法急性脑梗死患者230例,应用颈动脉彩色多普勒超声检查颈动脉内中膜厚度(IMT),分为不稳定斑块组96例、稳定斑块组71例、无斑块组63例;选择75例健康者作为对照组;检测各组血清WBC、单核细胞、hs-CRP及IL-6水平。结果急性脑梗死患者无斑块组的血清WBC、单核细胞、hs-CRPI、L-6水平分别与不稳定斑块组和稳定斑块组比较,差异有统计学意义(均P<0.01);不稳定斑块组hs-CRP及IL-6水平与稳定斑块组比较,差异有统计学意义(均P<0.01)。结论炎症反应是影响颈动脉粥样斑块稳定性的重要原因。

Objective To explore the correlation of inflammation and destabilization of carotid plaque with acute ischemic stroke.Methods 230 patients with acute brain infarction included 96 patients with destabilization of carotid plaque group（A group）,71 patients with the stabilization of carotid plaque group（B group） and 63 patients without carotid plaque（C group） according to the thickness of intima-media（IMT） evaluated by duplex Doppler ultrasound in the 230 patients with acute brain infarction.75 health people were normal controls.The levels of leucocyte（WBC）,monocyte,high sensitivity C-reactiveprotein（hs-CRP） and interleukin-6（IL-6） were measured.Results The distribution of WBC,monocyte,hs—CRP and IL-6 was significant different between C group and A group and between C group and B group（P0.01）.The distribution of hs-CRP and IL-6 was significant different between A group and B group（P0.01）.Conclusion These results suggest that inflammation plays a key role on carotid plaque destabilization.
