摘要
目的研究缺血预处理对肌皮瓣缺血再灌注损伤的保护作用并对其作用机制进行初步探讨。方法于家猪胸背部形成以胸背血管束为蒂的岛状肌皮瓣,对比观察缺血预处理后缺血再灌注肌皮瓣在缺血期、再灌注期的组织学变化,同时测定缺血、再灌注不同时间血清及肌肉组织中丙二醛含量。结果预处理组缺血期及再灌注期组织损伤均轻于对照组,其再灌注1,3h,肌皮瓣中丙二醛含量亦明显低于对照组(P<001)。结论缺血预处理可减轻肌皮瓣缺血再灌注损伤,其作用机制可能与其可诱导内源性超氧化物歧化酶活性增高、加快清除介导缺血再灌注损伤的氧自由基有关。
Objective To explore the protective effect and its possible mechanism of ischemic preconditioning onischemia induced reperfusion injury of myocutaneous flap. Methods The morphological changes of muscle was observed in the latissimus dorsal island myocutaneous flap of porcine model, the content of malondialdehyde(MDA) in serum and muscle homogenates were examined during ischemia and reperfusion. Results (1)The degree of the ischemia induced reperfusion injury of myocutaneous flap was significantly milder in the experiment (IP+I/R) group than in the control (I/R) one at 3 hours after ischemia and 3 hours after reperfusion. (2)Compared with the control group, the MDA content in muscle homogenates was lower at 1 and 3 hours after reperfusion( P <0 01) in the IP+I/R group. Conclusion Ischemic preconditioning can reduce the myocutaneous flap injury from ischemia and reperfusion, and its possible mechanism may be that the SOD could be induced and the O · 2 scanvenged rapidly after ischemic preconditioning.
