摘要 为进一步认识非酒精性脂肪肝(non-alcoholic fatty liver disease,NAFLD)的发病机制,寻求NAFLD临床最佳疗效依据,本文作者对近年来NAFLD的流行病学、发病机制及治疗进行了归纳和分析整理,发现NAFLD的发病与诸多因素有关,其中最重要的是"二次打击",还有各种损害因子,包括类固醇调节元件结合蛋白(SREBP)-1c和瘦素、肿瘤坏死因子(TNF-α)、脂联素、细胞性内毒素等细胞因子,共同促进了NAFLD的发展。针对发病机制临床治疗包括常规改善胰岛素抵抗、改善脂质代谢外,还包括肝细胞保护剂、抗氧化剂、抗毒性细胞因子等。 To find out the pathogenesis of non-alcoholic fatty liver disease(NAFLD),and to explore the basis for the best clinical curative effect,the author,based on an analysis of epidemiology,pathogenesis and treatment of NAFLD,found that the pathogenesis of NAFLD are attributed to many factors,of which the most important is "the second blow",along with all sorts of damage factors,including cytokines such as steroids regulating element binding proteins(SREBP)-1c and leptin,tumor necrosis factor(TNF alpha),adiponectin,cell endotoxin etc.According to the pathogenesis,clinical treatment includes regular improvement of insulin resistance and lipid metabolism,liver cell protectant,antioxidants,resisting toxicity,etc.
