2型糖尿病无症状左室舒张功能障碍与HCY、NT-proBNP及hs-CRP的关系

The Relationship Among HCY、NT-proBNP、CRP and Asymptomatic Left Ventricular Diastolic Dysfunction in Patients with Type Ⅱ Diabetes

摘要 目的测定无症状左室舒张功能障碍Ⅱ型糖尿病患者及对照组血浆同型半胱氨酸(Homocysteine,Hcy)、氨基末端脑钠肽前体(N-terminal brain natriuretic peptide,Nt-proBNP)和超敏C反应蛋白(high sensitivity C reactive protein,hs-CRP)水平,研究Hcy与后两指标的相关性。方法比较2型糖尿病无症状左室舒张功能障碍(N-LVDD TD)50例和对照组(CG)30例患者间BNP、Hcy、hs-CRP水平的相关性。采用SPSS13.0统计软件包进行统计分析。结果 N-LVDD TD血清NT-proBNP、Hcy、hs-CRP水平均较CG升高且有统计学意义。N-LVDD TD(r=0.775)组血清NT-proBNP与Hcy成正相关,与对照组比较均有显著性差异。结论 hs-CRP虽能有效检测出无症状左室舒张功能2型糖尿病患者,但是受多种因素的影响,特异性很低。NT-proBNP联合Hcy检测敏感性和特异性均很高,对临床早期发现治疗无症状左室舒张功能2型糖尿病患者有重要意义。 Objective To determine the left ventricular diastolic dysfunction in asymptomatic type Ⅱ diabetic patients and the control group plasma homocysteine(Homocysteine,Hcy),N-terminal brain natriuretic peptide(N-terminal brain natriuretic peptide,NT-proBNP)and high sensitive C-reactive protein(high sensitivity C reactive protein,hs-CRP)levels of Hcy and the relevance of the latter two indicators.Methods Compare the level of correlation of NT-proBNP,Hcy and hs-CRP in left ventricular diastolic dysfunction without symptoms of 50 cases of type Ⅱ diabetes and control group of 30 patients.SPSS 13.0 statistical package used for statistical analysis.Results The serum NT-proBNP,Hcy,hs-CRP levels of N-LVDD TD increase with statistical significance compared with CG.N-LVDD TD(r=0.775) serum levels of NT-proBNP were posilively correlated with Hcy,there were significant differences between NT-proBNP and Hcy of the expezimental group and the control group.Conclusion Although hs-CRP can effectively detect asymptomatic left ventricular diastolic dysfunction in patients with type 2 diabetes,but is affected by many factors and specificity is very low.The sensilivily and specificity for NT-proBNP joint Hcy is very high,and it is important for detection and clinical treatment of early asymptomatic ventricular diastolic function in patients with type Ⅱ diabetes.
