胆管镜在胆管手术中的临床应用

Clinical application of biliary duct mirror in biliary duct surgery

目的：探讨胆管镜（biliary duct mirror,BDM）在胆管手术中应用的价值。方法：回顾性分析我院2007~2010年应用（Oly-musCHF-P20型）BDM在胆管外科手术中500例患者的临床资料。结果：术中BDM检查结果显示肝内外胆管结石363例,其中肝外胆管结石225例,结石取净222例,取净率99.0%（222/225）;肝内外胆管多发结石98例,结石取净93例,取净率95%;单纯肝内胆管结石41例,结石取净38例,取净率94%（38/41）。术后肝外胆管结石残石率0.89%（2/225）,肝内胆管结石残石率10.2%（10/98）。结论：术中BDM检查直观、安全高效,具有诊断治疗双重作用,值得推广应用。

Objective：To investigate the application value of biliary duct mirror（BDM） in biliary duct surgery.Methods：The clinical data of 500 patients undergoing biliary duct surgery by OlymusCHF-P20 in our hospital from 2007 to 2010 were retrospectively analyzed.Results：The detection results of intraoperative BDM demonstrated 363 cases of intrahepatic or extrahepatic biliary tract stones.Among them,225 cases were extrahepatic biliary tract stones and all stones were removed in 222 cases with the removal rate of 99.0%（222/225）;98 cases were intrahepatic or extrahepatic biliary tract multiple stones and all stones were removed in 93 cases with the removal rate of 95%;41 cases were pure intrahepatic biliary duct stone and all stones were removed in 38 cases with the removal rate of 94%（38/41）.After operation,the residual stone rate of extrahepatic biliary tract stones and intrahepatic biliary tract stones was 0.89%（2/225） and 10.2%（10/98） respectively.Conclusion：Intraoperative BDM inspect is intuitive,safe and highly effective with the double action of diagnosis and treatment,which is worth promoting the application.
