鼻咽癌患者T细胞、巨噬细胞及红细胞免疫功能研究

Research on T lymphocyte subtypes and the phagocytosis activity of macrophages and the erythrocyte immune function in the patients with nasopharyngeal carcinoma

目的 探讨鼻咽癌患者体内多种免疫效应细胞功能的变化规律及其临床意义。方法 分别采用ＡＰＡＡＰ法、斑蝥皮泡法、免疫粘附法及酶免法检测了２９ 例鼻咽癌患者Ｔ细胞亚群、巨噬细胞吞噬率（ＰＲ）及吞噬指数（ＰＩ）、红细胞免疫粘附功能及血清ｓＩＬ２Ｒ水平，并分析其相互关系。结果 鼻咽癌组病人ＣＤ＋３ 细胞、ＣＤ＋４ 细胞、ＣＤ＋４ ／ＣＤ＋８ 比值降低、ＣＤ＋８ 细胞升高，与对照组比较有显著差异（Ｐ＜ ０．０１）；鼻咽癌患者ＰＲ与ＰＩ均显著低于对照组（Ｐ＜ ０．０１）；鼻咽癌组红细胞Ｃ３ｂ 受体花环率（Ｃ３ｂ ＲＲ）显著低于对照组（Ｐ＜ ０．０１），循环免疫复合物花环率（ＩＣＲ）则显著高于对照组（Ｐ＜ ０．０１）；鼻咽癌患者血清ｓＩＬ２Ｒ明显升高。Ⅲ期以上的鼻咽癌患者与Ⅱ期以下组比较上述免疫指标均有显著差异（Ｐ＜ ０．０１）。鼻咽癌组Ｃ３ｂ ＲＲ与ＰＲ及ＣＤ＋４ ／ＣＤ＋８ ，ＰＲ与ＣＤ＋４ ／ＣＤ＋８ 正相关。ｓＩＬ２Ｒ与Ｃ３ｂＲＲ、ＰＲ及ＣＤ＋４ ／ＣＤ＋８ 均显著相关（Ｐ＜ ０．０５）。结论 鼻咽癌患者多种免疫效应细胞功能下降，并随病情进展，免疫功能进一步下降。

? Objective To detect T lymphocyte,macrophages and erythrocyte immune function in patients with nasopharyngeal carcinoma(NPC).Methods T lymphocyte subtypes including CD + 3、CD + 4、CD + 8 and red blood cell immune function and phagocytosis rate(PR) and index(PI) of macrophages were measured in 28 NPC patients.Results The values of CD + 3、CD + 4、CD + 4/CD + 8 were lower and CD + 8 were higher in the NPC patients than those in nomal control (P<0.01);The values of C 3b RR was lower and ICR were higher in patients than in controls (P<0.01);The values of PR and PI were lower in patients than those in controls(P<0.01);The values of C 3b RR and PR and CD + 4/CD + 8 were closely related in patients with NPC (p<0.05).Conclusion T lymphocyte immune function,the phagocytosis activity of macrophages and erythrocyte immune function in NPC patients were significantly decreased,the changes of values of the three systems were closely interrelated. 〔
