摘要
【目的】探讨原发性肝癌CT动脉期强化程度与经肝动脉碘化油栓塞化疗(transarterial oily chemoembolization,TOCE)的碘化油沉积的相关性。【方法】对2003年1月-2009年1月共69例原发性肝癌患者术前肿瘤CT动脉期血供强化程度进行分型,将其分为富血供型(均匀强化型+不均匀强化型)和乏血供型(强化不明显型+无强化型),并与TOCE术后碘化油沉积情况进行相关性分析。【结果】69例行TOCE治疗的原发性肝癌患者中,CT动脉期表现为富血供型40例(均匀强化型10例+不均匀强化型30例),乏血供型29例(强化不明显型16例+无强化型13例)。四种分型碘化油沉积率的总构成比有显著性差异(χ2=60.096,P<0.01),肿瘤强化程度与碘化油沉积率呈显著正相关(r=0.704,P<0.01),富血供的碘化油沉积率明显优于乏血供(χ2=43.281,P<0.01);富血供中均匀强化型碘化油沉积率优于不均匀强化型。【结论】原发性肝癌CT动脉期强化程度与TOCE碘化油沉积具有相关性,利用CT动脉期进行原发性肝癌血供评估,可以初步预测原发性肝癌TOCE后的碘化油的沉积情况。
【Objective】 To assess the correlation between the degree of lesions enhancement of the CT hepatic arterial phase and lipiodol deposition of transarterial oily chemoembolization(TOCE) for primary liver cancer.【Methods】 69 patients with primary liver cancer were classified as hypervascular(homogeneous and heterogeneous) or hypovasccular(homogeneous and heterogeneous) based on the degree of lesions enhancement during the hepatic arterial phase before TOCE from Jan.2003 to Jan.2009.The correlation between the vascular pattern of primary liver cancer on the basis of the degree of lesions enhancement during the hepatic arterial phase and the lipiodol deposition of TOCE is analyzed.【Results】 We classified 69 cases with primary liver cancer as 40 cases of hypervascular(10 cases of homogeneous and 30 cases of heterogeneous) and 29 cases of hypovasccular(13 cases of homogeneous and 19 cases of heterogeneous) based on the degree of lesions enhancement during the hepatic arterial phase before TOCE.A statistically significant correlation between the hypervascular pattern on the basis of the degree of lesions enhancement during the hepatic arterial phase and the lipiodol deposition of TOCE was seen(r=0.704,P0.01).The lipiodol deposition of 4 vascular patterns is significant different(χ2=60.096,P0.01),the lipiodol deposition of hypervascular is significant better than that of hypovasccula(χ2=43.281,P0.01),the lipiodol deposition of homogeneous hypervascular is better than that of heterogeneous hypervasccula.【Conclusion】 The vascular pattern of primary liver cancer on the basis of the degree of lesions enhancement during the hepatic arterial phase was relative to the lipiodol deposition of TOCE.The vascular pattern of primary liver cancer based on CT arterial-phase scans may be helpful in predicting the lipiodol deposition of TOCE.
