广泛性焦虑与强迫症患者条件负变化对照研究 被引量:2

A comparative study of contingent negative variation between patients with generalized anxiety disorder and obsessive compulsive disorder

摘要 目的:用新的分析软件验证彩照诱发条件负变化(CNV)在广泛性焦虑(GAD)和强迫症(OCD)中的临床价值。方法:应用美国NicoletBravo脑诱发电位仪,采用彩照诱发以及反应时间技术,记录了41例GAD和69例OCD患者的CNV,并与58名正常对照者(NC)CNV进行比较。结果:①OCD组波形不规则,CNV-Ⅱ型尤甚;②OCD组、GAD组及NC组在命令信号后负变化(PINV),波幅M1上差异有显著性(P<0.01);与NC组相比,GAD组波幅M1降低,OCD组波幅M1增高;③与NC组相比,OCD和GAD组PINV出现率明显增多,OCD组最高,GAD组其次。结论:新软件为CNV临床应用提供强有力的技术支持。CNV可能是评定GAD和OCD大脑综合功能的有效工具之一,OCD患者CNV变化诸特点值得跟踪随访。 Objective: To assess the clinical value of contingent negative variation (CNV) in diagnosing generalized anxiety disorder (GAD) and obsessive compulsive disorder (OCD) by a new analytic software. Method:Event-related potentials CNV of 41 GAD,69 OCD patients and 58 normal controls were recorded using a Nicolet Bravo Instrument and compared among them. Results:①OCD patients presented irregular CNV wave,especially in CNV-Ⅱ type.②A significant difference of CNV was found among 3 groups,in both PINV and amplitudes of M1 (P0.01);compared with NC,GAD patients showed decreased M1 amplitude,and OCD patients showed increased M1 amplitude.③Compared with NC,two patient groups had increased ratio of PINV appearance,mostly in OCD patients,secondly in GAD patients. Conclusion:The new analytic software can provide strongly technique support for application of CNV to clinical diagnosis.CNV may be a useful instrument for assessing the brain function of GAD and OCD.The characteristics of CNV in OCD patients deserve further studying.
