摘要
[目的]了解屏南县流行性脑脊髓膜炎(流脑)带菌状况和免疫抗体水平,预测流脑发病的趋势,为流脑的防控工作提供科学依据。[方法]2009年12月采取分层随机抽样方法采集0~、3~、7~19、≥20岁共4个年龄组健康人群的咽拭子和静脉血,检测脑膜炎奈瑟菌(Nm)IgG抗体。培养采用常规方法,抗体水平测定用ELISA方法。[结果]调查188名健康人群流脑带菌率为零。抗A群Nm抗体阳性率为92.02%,几何平均浓度(GMC)为7.46μg/ml;抗C群抗体阳性率为48.4%,GMC为2.39μg/ml,男女比较抗体浓度和阳性率差异无统计学意义(P〉0.05)。不同年龄组A群抗体阳性率差异无统计学意义(P〉0.05),C群抗体差异有统计学意义(P〈0.05),3~6岁年龄组抗体阳性率最高,0~2岁年龄组最低。A和A+C群流脑疫苗均有接种的A群抗体、C群抗体阳性率均最高,都未接种者阳性率最低,部分无免疫史的人群也检出相应抗体,提示存在Nm自然感染的危险。[结论]屏南县不同年龄组健康人群A群流脑IgG抗体的阳性率维持在较高水平,而C群抗体阳性率较低,应加强流脑疫情监测,普及A+C流脑疫苗的免疫接种,防止流脑的暴发和流行。
[Objective]To understand the carrier status of epidemic meningitis virus and immune level among healthy people in Pingnan county,to predict the trend of the prevalence of epidemic meningitis,to provide a scientific basis for disease prevention and control.[Methods]Throat swab samples and venous blood were collected from the healthy people aged from 0-2,3-6,7-19 and ≥20 in December 2009 with stratified random sampling method,Neisseria meningitides(Nm) and IgG antibody were tested,the culture of Nm was conducted with common method.and the level of antibody was tested with ELISA method.[Results]Of the188 healthy people investigated,the carrier status of meningitis virus was zero,anti-A antibody positive rate was 92.02%,GMC was 7.46μg/ml;anti-C antibody-positive rate was 48.4%,GMC was 2.39μg/ml,There was no difference of the antibody concentration and positive rate between male and female(P0.05).There was no difference of the positive rate of A group in different age groups(P0.05),There was a difference of positive rate of C antibody in different groups(P0.05),The positive rate was the highest in the people aged from 3-6 well the lowest in the people aged from 0-2.The positive rate of anti-A antibody,C antibody was the highest in the people immunized with A and A+C vaccine well the positive rate was the lowest in the people without immunization with above vaccines.The antibody was also detected from some people,suggesting the risk of natural infection of Nm.[Conclusion]The positive rate of IgG antibody of group A epidemic meningitis remained at a high level among the healthy people in different aged groups in Pingnan county,well the positive rate of C group was low,surveillance should be enhanced,immunization of A+C vaccine should be given in order to prevent the outbreak and prevalence of epidemic meningitis.
