经脐单孔腹腔镜胆囊切除术35例报告

Transumbilical single-port laparoscopic cholecystectomy:a report of 35 cases

目的：探讨用普通器械完成经脐单孔腹腔镜胆囊切除术的可行性及安全性。方法：回顾分析2010年3月至10月为35例患者行经脐单孔腹腔镜胆囊切除术的临床资料。结果：全组均成功完成手术，手术时间35～11Omin，平均45．3min，术中出血5～20ml，平均8．5ml，无并发症发生。术后2～4d出院。术后1个月复诊，无不良后果，疤痕隐蔽，基本达到腹壁“无疤痕”的效果，患者满意。结论：用普通腹腔镜器械施行经脐单孔腹腔镜胆囊切除术是安全可行的，美容效果明显；手术巧用脐环作为镜孔，既防漏气又抗干扰，超声刀的使用降低了手术难度，并缩短了手术时间。

Objective：To investigate the feasibility and safety of transumbilical single-port laparoscopic cholecystectomy using ordinary instruments. Methods： From Mar. 2010 to Oct. 2010,35 cases of transumbilical single-port laparoscopic cholecysteetomy were performed. The clinical data and surgical outcomes were retrospectively reviewed. Results ： All operations were successful. Operative time was 35 to 1lOmin,average 45.3rain;blood loss was 5 to 20ml, average 8.5ml. No complications happened. The postoperative hospital stay was 2 to 4d. Reexamination was performed 1 month later, no side effect was found, and the abdominal wall scar was minimal. No surgical scars could be observed on the abdomen and patients were satisfied with it. Conclusions：Transumbilical single-port laparoseopic cholecystectomy using usual laparoscoplc instruments can be completed safely with obvious cosmetie effects. The umbilical ring is used as laparoscopic hole in the operation,which prevents gas leakage and interference. Application of ultrasound knife can reduce the difficulty of operation and shorten the operative time.
