影像学检查在胃间质瘤的术前诊断价值

Value of imaging examination on preoperative diagnosis of gastric stromal tumor

目的：分析胃间质瘤患者各种相关影像学检查的临床价值。方法：回顾性分析1999年～2009年收治的42例胃间质瘤患者的临床资料。结果：42例胃间质瘤患者平均年龄57．4岁，其中〉150岁者32例，占76．2％。男28例，女14例，男女之比2：1。B超是最常用的检查方法，但阳性率不高（22％）；上消化道钡餐造影定位诊断较为准确；CT和MRI检查较少采用，但有助于肿瘤性质的判断，并可提示有无转移病灶。全组术前诊断胃肿瘤者38例，其中诊断为胃间质瘤（恶性）者仅1例。结论：胃间质瘤多发于老年人，以男性多见，临床上缺乏特征性的表现。应有选择地采用影像学辅助检查，以提高术前确诊率。

Objective To study the clinical significance of imaging examinations on the preoperative diagnosis of gastric stromal tumor. Method A retrospective study of clinical material of 42 cases of gastric stromal tumor between 1999 and 2009 was made. Results The average age was 57.4 （ 36 - 77 ） years, 32 cases were at or over 50 years, contributed to 76. 2% of all 42 pa- tients. Twenty -eight were male and 14 were female,the male/female ratio was 2： 1. Ultrasonic examination was most commonly used, but its positive rate was not high （22%）. On tile digestive tract barium meal radiography localization diagnosis was more accurate;CT and MRI were not often taken, but were helpful for differentiation of tumor feature, and sometimes they gave clues of metastasis. The pre - operative diagnosis of 38 cases was gastric tumor. Among them,only one patient was diagnosed as gastric stromal tumor （malignant）. Conclusion - Gastric stromal tumor is rare, occurs predominantly in old patient and male patient. It lacks of specific clinical manifestation. Assisted examination should be chosen appropriately to increase the accuracy of diagnosis.
