初治儿童及成人伴T系抗原CD2表达的B-ALL的免疫表型分析

摘要 目的了解儿童及成人伴CD2阳性表达B系急性淋巴细胞白血病(CD2+B-ALL)的免疫表型特征,为临床的诊断、治疗及预后判断提供依据。方法 应用流式细胞术及多种单克隆抗体检测了21例儿童及22例成人的CD2+B-ALL免疫表型,并对结果进行分析比较。结果 儿童及成人的CD2+B-ALL病例没有明显的性别差异;两者的免疫标记比较,成人CD2+B-ALL的CD10表达明显高于儿童,且差异显著(P<0.01),CD34表达则明显低于儿童,有显著差异(P<0.01);且不管是儿童还是成人CD2+B-ALL都高表达CD34;但两组伴髓系抗原及CD34阳性率均没有显著差异(P>0.05)。结论 儿童及成人CD2+B-ALL主要来源于造血干/祖细胞的恶性转化且儿童病例CD34的表达高于成人,其细胞抗原的异质性表达可能与它的早期起源有关。 Objective To study the immunophenotypic feature of CD2+ adult and childhood B cell acute lymphoblastie leukemia ( CD2+ B - ALL) and provide evidences for the diagnosis ,therapy and prognosis. Methods The immunophenotypes of 21 casese of childhood and 22 cases of adult CD2+B cell acute lymphoblastic leukemia (CD2+B -ALL) were assayed by a panel of monoelonal antibodies(McAbs) with FACS. Results There was no gender difference between adult and childhood CD2 + B - ALL. The expression frequencies of CD10 was higher (P 〈0.01 ), and CD34 were lower(P 〈 0.01 ) significantly in the adult grou. Both groups showed high expression frequencies for CD34. The incidence of myeloid - associated antigen( CD13, CD33 ) and CD34 expression were no difference(P 〉 0. 05 ). Conclusion The a- dults and childhood CD2 ~ B - ALL might originate from the malignant transformation of early hemotopoietic stem cells. The indidenee of CD34 expression is higher in childhood than in adults patients.
