目的:评价、推广ABO疑难血型3步分析法在正反定型不一致导致疑难血型鉴定中的应用。方法:对167例因正反定型不一致导致疑难血型鉴定的样本,应用ABO疑难血型3步分析法进行分析、鉴定、评价应用效果。结果:32例属于人为原因操作不规范导致正反定型结果不一致,135例疑难血型鉴定分析原因主要为:生理因素、自身抗体、不规则抗体、血浆蛋白异常、血液病等影响因素。按照不同的确诊试验分别进行鉴定分析判断。结论:ABO疑难血型3步分析法规范了实验室疑难血型鉴定步骤,缩短了试验时间,准确率高,为挽救临床受血者的生命赢得了宝贵的时间,值得向临床推广应用。

Objective：To evaluate and popularize the three-step analysis in the complicated ABO blood group resulted from the inconsistency in forward and reverse serotyping of ABO blood group.Method：167 complicated samples resulted from the inconsistency in forward and reverse serotyping were analyzed,identified and evaluated with three-step analysis.Result：167 complicated blood samples were gaven to the right judgments.The cause of inconsistency in forward and reverse serotyping in 32 samples was man-made.The main cause of 135 samples were physical factors,autoantibodies,irregular antibodies,plasma protein abnormalities and hematologic diseases.All diagnosis were identified according to different test.Conclusion：The three-step analysis of the complicated ABO blood group would standardize laboratory procedure,shorten the test time and improve accuracy,which is worthy to clinical application.
