摘要
目的探讨实时三维超声对胎儿唇腭裂畸形的早期诊断价值。方法应用GE Volusion E8 Expert三维容积探头对2008年8月以来在我院行实时三维超声检查的2290例18~37周胎儿唇部进行观察。结果 2290例中应用三维超声能观察到胎儿唇部的有2287例,看不到胎儿唇部的有3例,总检出率998%,胎儿面部正常2244例,唇裂畸形43例,其中有32例伴牙槽突裂及腭裂,1例单纯腭裂漏诊,畸形率1.92%,畸形检出率97.7%,漏诊率0.04%。以上畸形病例均在我院生产或引产后得到证实。结论常规二维超声产前对胎儿颜面部大部分表面结构及深部骨性结构均能清楚显示,实时动态三维超声能更加详细全面且直观地提供胎儿形态信息及运动状况,可对二维超声加以补充,在胎儿产前唇腭裂早期诊断中具有重要的应用价值。
Objective: To explore the early diagnostic value in early pregnancy of real-time three dimensional ultrasound to cleft lip and palate malformation in fetus.Methods: The routine obstetric real – time three dimensional ultrasound records were retrospectively reviewed on total of 2290 fetuses (18-37week gestational age) by using GE Volusion E8 Expert three dimensional volume probe to observe lips in our horspital since August 2008.Results: Of the 2290 Fetuses.Fetal lips in 2287 cases were seen and not in 3.the total detection rate reached 99.8%.Fetus face was normal in 2244 cases.Cleft lip was 43cases.32 cases were accompanied with cleft alveolar process and palate.1 pure palate was missed to recognize.The abnormal rate was 1.92%.The abnormal detection rate was 97.7%.The false negatine rate was 0.04%.The diagnoses above abnormal cases were confirmed after subsequent labor or induced labor.Conclusion: The majority of Superficial facial anatomic structures as well as the deep bony structures of the fetal face were clearly displayed by conventional two dimensional ultrasound in the prenatal.Real-time three dimensional ultrasound can provide more abundant and direct information of fetal surface imaging and movement status.It is a complement to conventional two dimensional ultrasound.It has an important practical clinical value in early prenatal diagnosis of fetal cleft lip and palate malformations.
