摘要
目的探讨探讨α-平滑肌肌动蛋白(α-smooth muscle actin,α-SMA)在口腔鳞状细胞癌(oral squamous cell carcinoma,OSCC)间质成纤维细胞中的表达及其与口腔鳞状细胞癌临床病理特征的关系。方法收集存档的OSCC石蜡组织标本80例,以正常口腔黏膜5例、癌前病变5例为对照,制作组织切片后,以LsAB法行α-SMA免疫组化染色,检测α-SMA在口腔鳞状细胞癌及正常粘膜和癌前病变中间质成纤维细胞中的表达特点,并对其与口腔鳞状细胞癌临床病理特征的关系进行统计分析。结果α-SMA在5例正常口腔黏膜和5例癌前病变的间质成纤维细胞中表达阴性;在80例OSCC标本中,间质成纤维细胞阳性表达61例,其中,45例病理分级Ⅰ级中有29例阳性表达,35例Ⅱ~Ⅲ级OSCC中阳性表达32例,随着细胞分化程度的逐渐降低,α-SMA平均染色阳性标本百分率有逐渐升高的趋势(P<0.05);α-SMA在无淋巴结转移的42例患者中30例呈阳性表达,有淋巴结转移的38例患者中31例呈阳性表达,α-SMA阳性表达率与患者的颈淋巴结阳性转移率无显著相关性(P>0.05)。结论口腔癌间质成纤维细胞是有平滑肌分化的特征。
Objective To investigate the expression of α-smooth Muscle Actin (α-SMA) in stromal fibroblasts of oral squamous cell carcinoma (OSCC), and to analyze the correlation between the expression of α-SMA and clinicopathologic characteristics of OSCC.Methods Eighty cases of OSCC, 5 cases of OLP/OLK and 5 cases of normal oral mocosa were detected for the expression of α-SMA in the stroma by immunohistochemical method.Results Percentage of positive cases of α-SMA was increased with the decreased level of cell differentiation in OSCC group (P0.05). However, no significant correlation was demonstrated between α-SMA expression and cervical lymph node metastasis (P0.05).Conclusion α- SMA expression in stromal fibroblasts of OSCC could be used as an index of OSCC in its supplementary diagnosis and severity judgement, and they may play a role in oral carcinogenesis.
