Treatment of traumatic carotid-cavernous fistula by using covered stent

目的总结应用覆膜支架治疗创伤性颈内动脉海绵窦瘘(TCCF)的临床经验,包括适应证、安全性、有效性和手术技巧。方法回顾性分析2004年3月至2008年12月应用Jostent覆膜支架治疗的7例TCCF患者的临床资料,包括年龄、部位、瘘口大小、临床症状和预后等。结果所有患者均成功置入覆膜支架,术后造影显示瘘口闭塞,供血动脉通畅,眼动脉保留。随访患者均无异常表现。结论覆膜支架是治疗TCCF安全有效的治疗手段,可作为可脱球囊栓塞失败患者的替代治疗方法。

Objective To summarize the clinical experiences in the treatment of traumatic carotid-cavernous fistula （TCCF） using covered stent-graft, including the indications, safety, efficacy, and surgical skills.Methods The clinical data of 7 cases of TCCF treated by Jostent covered stent from March 2004 to December 2008 were analyzed retrospectively, including age, location, fistula size, clinical symptoms and prognosis.Results Stent-graft was successfully implanted in all patients. Postoperative angiography showed the fistula occlusion, the patency of parent artery, and the preservation of ophthalmic artery. No abnormal findings were found in the follow-up. Conclusion Covered stent-graft is safe and effective in the treatment of TCCF, which can be used as an alternative in the patients who failed in the detachable balloon embolization.
