经阴道超声监测排卵在不孕症中的应用研究 被引量:11

摘要 目的经阴道超声监测卵泡生长、发育、排卵情况,评价临床用药治疗效果.方法对100例诊断为排卵障碍性不孕症经促排卵治疗者进行经阴道超声监测卵泡发育,从月经第10天开始监测卵泡发育,观察子宫内膜厚度、分期,卵巢内卵泡大小、数目、卵泡生长速度,是否排卵,指导临床治疗.结果 100例经促排卵治疗,卵泡成熟并排卵60例,卵泡不发育11例,卵泡闭锁9例,卵泡黄素化未破裂者5例,多囊卵巢11例,卵泡过度刺激4例.结论经阴道超声监测排卵并指导临床用药,对不孕症诊治具有非常重要的临床价值;经阴道超声监测卵泡优于临床根据基础体温和血及尿中激素水平的变化来估计排卵日期. Objective To monitor the growth and development of ovarian follicle with transvaginal ultrasound examination,and evaluate the therapeutic effect of drugs on ovulation.Mehods We monitored the development of ovarian follicle with transvaginal ultrasound examination in 100 women with infertility caused by ovulation failure who received ovarian stimulation treatment.We monitored the development of ovarian follicle,observed the thickness and stage of endometrium,the size,number and growth speed of ovarian follicle,and ovulation from the 10th day of menstruation,and guided for clinic therapy.Results Among the 100 patients with infertility caused by ovulation failure who received ovarian stimulation treatment,matured ovarian follicle and ovulation was found in 60 patients,ovarian follicular nondevelopment was found in 11 patients,follicular atresia was found in 9 patients,follicular luteinization without rupture was found in 5 patients,polycystic ovary was found in 11 patients,and ovarian hyperstimulation was found in 4 patients.Conclusions Monitoring ovulation with transvaginal ultrasound examination can guide for clinic drug use,and has important clinical value for diagnosis and treatment of infertility.Monitoring ovulation with transvaginal ultrasound examination is better than monitoring basal body temperature and serum and urinary hormone levels in predicting the ovulation date.
