药历在肝肾移植患者药学服务中的建立及应用

Establishment and Application of Medication History in Pharmaceutical Care for Patients Underwent Liver and Kidney Transplantation

目的:评价药历在肝肾移植患者药学服务中的作用。方法:通过介绍我院药历应用的格式,并采用论述、实例分析,对我院2000-2009年肝肾移植患者药历进行回顾性分析总结。结果与结论:药历是临床药师开展药学服务的必备资料,在促进临床药师与患者、临床医护人员的沟通交流,提高临床药师的综合素质和能力,推动合理用药工作开展方面发挥了重要的作用。药历的应用促进了我院药学服务的发展。

OBJECTIVE. To evaluate the role of medication history in pharmaceutical care for patients underwent liver and kidney transplantation. METHODS ： Through introducing application format of medication history in our hospital, discussing and analyzing case, the application experience of medication history in patients underwent liver and kidney transplantation from 2000 to 2009 in our hospital were retrospectively analyzed and summarized. RESULTS＆CONCLUSION： Medication history is necessary for clinical pharmacist to carry out pharmaceutical care. Medication history has played an important role in promoting communica- tion between clinical pharmacists and patients, clinical staff, improving the overall quality and ability of clinical pharmacists, pro- moting the rational use of drugs. The application of medication history promotes the development of pharmaceutical care in our hospital.
