心电散点图呈现的房室结功能不应期及对宽QRS波的鉴别 被引量：26

摘要 心电散点图亦称RR间期散点图,其二维直角坐标系中的每个散点所对应的横、纵坐标是相邻两个RR间期值。心房颤动(简称房颤)的散点图呈现了动态房室结功能不应期(AVNFRP)的电生理特征,可用于判断房室分离和宽QRS波的鉴别诊断:房颤时最短的RR间期可以作为AVNFRP的估算值,房颤扇形散点分布的图形边缘是动态AVNFRP界线(简称界线)。经房室结下传的室上性心搏散点都分布在扇形之中,而室性宽QRS波的散点可分布在扇形之外,是房室分离的散点图特征。组成"界线"的各散点所代表的室上性RR间期有心率依赖性,而室性联律间期无明显心率依赖性,二者有明显差异,也是房室分离的重要依据。房颤伴差异性传导、房颤伴预激、窦性心律时的差异性传导与室性早搏都有特异性散点图形。提示心电散点图对动态心电数据中宽QRS波的鉴别诊断具有优势。
