全凭七氟醚麻醉在老年腹腔镜手术中对循环功能的影响

The Effect of Depends on Sevoflurane Anesthesia on Function of Cycle for the Elderly Patients in Laparoscopic Surgery

目的探讨全凭七氟醚麻醉在老年腹腔镜手术中对循环功能的影响。方法 2009年5月～2010年6月我院择期行腹腔镜手术的老年患者86例,采用全凭七氟醚麻醉的方法,观察并记录诱导前、诱导后2min、插管即刻、插管后5min、气腹完毕时的HR、SBP、DBP、MAP等血液动力学指标以及BIS的变化。结果与诱导前相比,插管即刻的HR、SBP、DBP和MAP均明显提高,差异具有统计学意义(P<0.05);与诱导前相比,人工气腹完毕后SBP、DBP和MAP亦明显提高,差异具有统计学意义(P<0.05)。而其他各时点血流动力学参数变化以及BIS变化不明显,差异不具有统计学意义(P>0.05)。另外,患者的平均苏醒时间为(11.09±4.23)min。结论全凭七氟醚麻醉能够减小腹腔镜手术对老年患者血流动力学和呼吸功能的影响,为提高患者手术的生活质量奠定了基础,从而具有重要的临床应用价值。

Objective To probe into the effect of depends on sevoflurane anesthesia on function of cycle for the elderly patients in laparoscopic surgery. Methods From May 2009 to June 2010, 86 cases of elderly patients with depends on sevoflurane anesthesia for laparoscopic surgery in our hospital were selected. HR, SBP, DBP, MAP of before induction, after 2 min, tracheal intubation, 5 rain after extubation, time when completed pneumoperitoneum and other hemodynamic indices and BIS changes were observed and record- ed. Results Compared with those before induction, HR, SBP, DBP and MAP of tracheal intubation were markedly increased and the difference was significant （P〈0.05）. SBP, DBP and MAP after artificial pneumoperitoneum also markedly increased and the difference was statistically significant （P〈0.05）. But other hemodynamic parameters at each time point and BIS did not change significantly, and the difference was not statistically significant （P〉0.05）. In addition, the average recovery time of patients was （11.09± 4.23） rain. Conclusion Depends on sevoflurane anesthesia can reduce the influence of laparoscopic surgery on hemodynamic and respiratory function, improve the quality of life in elderly patients and has important clinical value.
