摘要
目的:探讨ω-3脂肪酸对慢性阻塞性肺病患者抗炎和免疫调节治疗的辅助作用。方法:将60例病人随机分为实验组和对照组。对照组采用常规治疗,实验组病人在常规治疗的基础上加用ω-3脂肪酸注射液(100 mL·d-1,iv),并于治疗前和治疗后第3天检测患者体内的C反应蛋白(CRP)、CD3、CD4、CD4/CD8及NK细胞的水平,记录病情严重程度(APACHE Ⅱ评分)、氧合指数(Pa02/FiO2)的变化、入住ICU时间和带呼吸机时间。结果:相对于对照组,实验组病人血清CRP明显下降(P<0.05),CD3、CD4、CD4/CD8及NK细胞水平显著高于对照组(P<0.01)、氧合指数改善(P<0.05),入住ICU时间减少(P<0.05)、使用呼吸机时间缩短(P<0.05);但在APACHE Ⅱ评分上,两组间无显著性差异。结论:应用ω-3脂肪酸可使慢性阻塞性肺病患者血清炎症介质表达减少,改善患者免疫功能和呼吸功能,缩短使用呼吸机时间和入住ICU时间。
Objective:To investigate the immune regulation mechanism and the effect of ω-3 fatty acids on patients with chronic obstructive pulmonary disease.Methods:60 patients were divided into two groups:test group and control group.The control group was given routine therapy while the test group was given ω-3 fatty acids injection(100 mL ·d-1,iv) simultaneously with routine therapy.For every patient the expression level of serum C-reactive protein(CRP),the serum level of CD3,CD4,CD4/CD8 and NK cells were determined at the beginning and on the third day after therapy,and the severity of the disease(APACHEⅡscore),theoxygenation index(Pa02/Fi02),the days needing to stay in ICU and the hours needing to use respirator were also recorded.Results:Compared with control group,the serum level of CRP of test group was significantly reduced,the serum level of CD3,CD4,CD4/CD8 and NK cells was increased,and oxygenation index was improved.The days in ICU and the hours of therapy with respirator in the test group were also reduced.However,there was no difference in the APACHE lI score between two groups.Conclusion:ω-3 fatty acids injection can decrease the inflammation mediators on patients with COPD and improve the cellular immune function.
