乳腺良性病变术后瘢痕组织的X线表现

Mammographic Appearance of the Postsurgical Breast Scar

目的回顾分析49例乳腺良性病变术后病例的X线平片,探讨乳腺术后瘢痕组织的X线特征及其与乳腺癌的鉴别诊断。方法评价49例乳腺术后随访中切口及附近出现致密影病例的平片者的临床资料、各种X线异常并与病理结果对照。结果 49例切口及附近均出现软组织致密影,伴钙化13例,结构紊乱18例,皮肤改变11例,脂肪坏死1例及异物影2例。49例重复活检结果均为良性的术后改变,其中包括:导管典型性及非典型性增生、导管内乳头状增生、导管扩张、硬化性腺病、大汗腺化生、异物反应、脂肪坏死及放射状瘢痕等。结论结合临床及X线平片特征,有助于对乳腺术后瘢痕组织做出正确的鉴别诊断,减少不必要的重复活检。

Objective A retrospective review of 49 mammographic examinations with dense located adjacent to biopsy site pro- vide the features helpful in differentiating postsurgical scarring from breast cancer. Methods Evaluation of 49 cases mammography with dense or speculated mass located adjacent to biopsy site was recorded. Mammographic and pathologic findings were cor- related in these cases. Results 49 case of parenchymal scar, with calcification （ n = 13 ）, architectural distortion （ n = 18 ）, skin changes （ n = 11 ）, fat necrosis （ n = 1 ）, opaque foreign body （ n = 2 ）. Re-excisiona] biopsy results ： In 49 cases, including one case of atypical ductal hypeplasia seen within the lesion, pathologic findings were benign. Conclusion Correlation of the patient＇ s clinical features with the mammographic findings can help radiologists to establish the differential diagnosis and to obviate unnec- essary re-excisional biopsy.
