实时超声弹性成像诊断老年人乳腺肿块良恶性的价值

The value of realtime ultrasonic elastography in differentiating the elderly breast malignant lesions from the benign

目的探讨利用实时超声弹性成像(弹性成像)诊断老年人乳腺肿块良恶性的价值。方法 2009年1月至2009年11月,162例患者187个乳腺肿块行常规超声检查和弹性成像检查,实时弹性成像结果评分参照改良5分法,将弹性成像结果与手术病理结果对照分析。结果恶性肿块55个,弹性成像评分结果:1分1个;2分5个,3分8个,4分24个,5分17个。良性肿块132个,弹性成像评分结果:1分68个;2分46个,3分9个,4分9个,5分0个,乳腺恶性肿块弹性评分与良性肿块弹性评分分布明显不同(P=0.000)。以弹性成像评分≥3分诊断恶性肿块,其敏感性、特异性、准确性分别为:89.1%,86.4%,87.2%。结论超声弹性成像鉴别老年人乳腺肿块良恶性具有较高临床应用价值。

Objective To evaluate the value of real-time ultrasonic elastrography（UE）in differentiating the benign and malignant elderly breast lesions.Methods 187 breast lesions of 162 patients were examined by gray and color sonography and UE,which is scored by revised 5-score method,and the results of EU were compared with pathology.Results UE result of 55 Malignant lesions：1-score with 1 lesion;2-score with 5 lesions;3-score with 8 lesions;4-score with 24 lesions;5-score with 17 lesions.UE result of 132 benign lesions：1-score with 68 lesions;2-score with 46 lesions;3-score with 9 lesions;4-score with 9 lesions;5-score with none.The distribution between malignant and benign lesions were different in UE-score（P=0.000）.The sensitivity,specificity and accuracy of UE defining malignant lesions with the UE score ≥3 were separately 89.1%,86.4%,87.2%.Conclusion Ultrasonic elastography is a good method of differentiating elderly breast malignant lesions from benign lesions.
