支气管舒张试验的结果判断与应用进展

Application Progress and the Result Judgement of Bronchodilatation Test

支气管舒张试验是判断呼吸道阻塞可逆性的客观指标,在支气管哮喘、慢性阻塞性肺疾病等呼吸道阻塞性疾病的诊断、鉴别诊断和用药疗效判断等方面发挥着重要的作用。但因长期以来各医疗单位所采取的支气管试验方法并不统一,目前国内外对支气管舒张试验阳性标准也不尽相同。我国2008年指南以FEV1增加≥12%,且FEV1增加绝对值≥200 mL为阳性标准。随着肺功能检测技术的飞速发展,脉冲振荡检测技术越来越受重视,研究表明脉冲振荡部分参数在支气管舒张试验中与FEV1有很好的相关性,其阳性标准正在探讨之中,是常规肺功能检查的必要补充手段。

The bronchodilatation test is the objective indicator to judge the airway obstruction reversibility and plays an important role in diagnosis,differential diagnosis and medicine efficacy determination of obstructive respiratory tract diseases,such as bronchial asthma,chronic obstructive pulmonary disease.However,the method of bronchus test which has been used by each medical institutions is not unified,and by now the positive criteria of the bronchodilatation test is different both home and abroad.The 2008 Guideline of China regards the increase of FEV1≥12%and the increased absolute value of FEV1≥200 mL as the positive criteria.With the rapid development of pulmonary function detection techniques,the impulse oscillation is more and more valued.The studies have shown that part of the impulse oscillation parameter whose positive criteria is still under discussion correlates with the FEV1 in the bronchodilation test and it must be the necessary supplement to the regular pulmonary function test.
