Relations of Pathology, Treatment and Prognosis in Infant Teratoma

目的探讨小儿畸胎瘤临床病理与治疗预后的关系。方法对377例小儿畸胎瘤的病理、AFP及随访，进行回顾性分析。结平恶性57例，占1512％。有6例病理报告为良性，术后复发为恶性。结去畸胎瘤的实质部分应现为潜在恶性;如有间变组织存在，应现为恶性;对AFP增高而病理报告为良性者，应仔细检查畸胎瘤组织中有无卵黄囊组织。早期应彻底切除肿瘤。对骶尾部隐匿型、混合型畸胎瘤，在切除肿瘤及尾骨的同时，还应切除1～2节骶骨。

Objective To study the connections of infant teratoma pathology and relative factors for prognosis as well as treatment. Methods Review and analyze the pathology, AFP and outcome of 377 infants teratoma cases. Results Of 377 cases, 57 were malignant, anounting for 15.12 %. Six cases were reported as benign teratoma but recurred to malignant after surgery. Conclusion Parenchymatous portion in teratoma should be considered as potentially malignant. If in direct metamorphosis tissues were found, then that should be considered as malignant teratoma. The benign teratoma as seen from pathological reports with upward AFP, should be checked very carefully to assure there were endodermal sinus tissue in teratoma tissues or not. Sacrococcygeal teratoma should be resected completely in initial stage. And one or two pieces of sacral should be resected in latent teratoma and teratomas with mixed pattern.
