社区居民脂肪肝患病率及影响因素分析

Prevalence and risk factors of fatty liver disease in community residents

目的分析温州市瓯海区社区居民脂肪肝的流行现状及其影响因素。方法采用整群抽样的方法抽取2个社区1 872名≥18岁的常住居民进行问卷调查、普通体检、超声波检查和实验室检测。用SPSS 13.0软件进行数据统计分析,计数资料用率来表示,组间比较用χ2检验;计量资料用均数±标准差(x±s)表示,组间比较用独立样本t检验;危险因素分析用非条件logistic回归分析,选用Forward LR逐步法,进入标准为0.05,剔除标准为0.10;P<0.05为差异有统计学意义。结果温州市瓯海区社区居民脂肪肝标化患病率为11.96%,其中男性为14.58%,女性为10.26%。脂肪肝患病率随年龄的增加总体呈上升趋势,脂肪肝组的体质指数(BMI)、腰围、收缩压(SBP)、舒张压(DBP)、血糖(FPG)、甘油三酯(TG)、血清胆固醇(TC)和低密度脂蛋白胆固醇(LDL-C)检测均值高于正常组,高密度脂蛋白胆固醇(HDL-C)检测均值低于正常组,差异均有统计学意义(P<0.05)。Logistic多因素逐步回归分析结果显示,腰围粗、体质指数大、高TC血症、高TG血症、低HDL-C血症、糖尿病、超重和肥胖以及中心性肥胖是脂肪肝患病的危险因素。结论瓯海区社区居民脂肪肝的患病率较高,肥胖、高脂血症及其相关的代谢紊乱与脂肪肝的患病关系密切。

Objective To investigate the prevalence and risk factors of fatty liver disease in the community residents of Ouhai District,Wenzhou.Methods A random cluster sampling method was applied to select two communities for the study.1 872 permanent residents aged over 18 years were chosen for a cross-sectional survey,questionnaire,physical examination,ultrasound and laboratory testing.Results The standardized prevalence rate of fatty liver was 11.96%（14.58% in men and 10.26% in women）.Fatty liver disease prevalence increased with age.The values of BMI,waist circumference,SBP,DBP,FPG,TG,TC and LDL-C were significant higher in the fatty liver disease group than that in the normal group（P〈0.05）.A logistic stepwise regression analysis showed that fatty liver disease was associated with waist measurement,body mass index,high TC hyperlipidemia,high TG hypertriglyceridemia,low HDL-C disease,diabetes mellitus,overweight,obesity and central obesity.Conclusion The fatty liver disease is prevalent in the community residents of Ouhai District.Obesity,hyperlipidemia and its related metabolic disorders were risk factors of fatty liver disease.
