彩色多谱勒血流显像诊断前列腺癌的价值

Color doppler flow imaging in the screeing diagnosis of prostate cancer

目的 了解彩色多谱勒血流显像在诊断前列腺癌中的价值。 方法 对８０ 例疑诊前列腺癌的患者，进行前列腺低回声区的血流显像检测和前列腺结节组织穿刺活检。 结果 全组血流显像率为６８ ％ （５４／８０） 。血流显像的５４ 例中６３ ％ （３４／５４） 为前列腺癌，前列腺增生占３７ ％ （２０／５４） 。经卡方检验两组的血流显像率差异有极显著性意义（ χ２ ＝ １５ ．９３ ，Ｐ＝ ０ ．０００１） 。血流显像的灵敏度为０ ．８９ ，特异度０ ．５２ ，阳性预测值为０ ．６３ 。 结论 前列腺结节的血流显像与前列腺癌有一定关系。直肠Ｂ 超探测前列腺结节时应加作结节的超声血流显像。

Objective To investigate the role of color doppler flow imaging in diagnosis of prostate cancer. Methods Transrectal ultrasound and color doppler flow imaging were conducted for 80 patients suspected of prostate cancer. All the patients underwent prostate biopsy. Results Among the 80 patients,42 were BPH (53%) and 38 were prostate cancer (47%). The total positive frequency of color doppler flow imaging was 68% (54/80).Of the 54 color doppler flow imaging positive cases 34 were malignant and 20 benign(χ 2=15.93, P =0.0001).The sensitivity of color doppler flow imaging was 0.89,the specificity 0.52 and the positive predictive value 0.63. Conclusions Color doppler flow imaging is helpful in the diagnosis of prostate cancer.
