椎-基底动脉延长扩张症患者脑血流动力学初探 被引量：8

摘要 目的研究椎．基底动脉延长扩张症（VBD）患者的脑血流动力学特征。方法采用经颅多普勒超声（TCD）连续观察18例经CT、MRI或磁共振血管成像（MRA）诊断的VBD患者（VBD组）的脑血流动力学特征，并与同期就诊的65例非VBD患者（非VBD组）进行对照。结果VBD组与非VBD组相比主要表现为椎动脉、基底动脉收缩期峰值血流速度减慢[（-41±16）cm／s比（-60±12）cnds，P=0．038]，收缩期峰值血流速度，舒张末期血流速度降低（1．51±0．65比2．51±0．74，P=0．026），而且，两组的峰值平均血流速度[（-28±9）ends比（-39±17）cm／s]、搏动指数（O．37±0．14比0．75±0．20）、阻力指数（0．52±0．12比0．78±0．16）比较差异均有统计学意义（P值分别为0．037、0．028、0．036）。VBD组TCD频谱形态表现为波峰圆钝，多数患者出现频窗充填。结论VBD患者的血流动力学表现具有相对的特异性，TCD可作为其临床初步筛查和辅助诊断手段之一。
