妇科腹腔镜手术中不同麻醉方法气腹和体位对呼吸功能的影响 被引量:10

摘要 目的:比较妇科腹腔镜手术中使用不同麻醉方法时气腹和体位对呼吸功能的影响,探讨麻醉合理的处理方法。方法:妇科腹腔镜手术患者800例,随机分为硬膜外麻醉组(EA)和全麻组(GA)各400例;ASAI-Ⅱ级,术中维持气腹压12-15 mmHg、20°T位。结果:EA组气腹及T位后VT、MV增加,PETCO2高于GA组,SpO2比GA组低。GA组气腹后PPEAK增加,C下降,T位后PPEAK进一步增加。EA组T位后动脉血pH值(〈7.350)比GA组低,气腹及T位后PaCO2明显高于GA组。结论:妇科腹腔镜手术气腹、T位使肺顺应性下降,肺有效通气量明显降低,EA下有发生高碳酸血症和低氧血症的趋势,采用GA则能有效避免,安全性相对较高。
