颈源性头痛临床诊断依据的分析研究

ANALYSIS OF CLINICAL DIAGNOSTIC CRITERIA FOR CERVICOGENIC HEADACHE

目的:分析颈源性头痛患者的临床症状、体征和影像学检查,讨论其临床诊断依据,为临床诊断提供参考指标。方法:分析2007年3月～2009年3月在我院收治的诊断为颈源性头痛的患者共108名,按就诊顺序分为A组和B组。对两组患者的年龄、性别、病程、头痛程度的VAS评分,头痛部位、头痛的性质、颈部僵硬感、颈部阳性体征及颈部影像学检查等临床表现进行logistic回归分析,以讨论其诊断依据。结果:在颈源性头痛的各项临床表现中,颈部僵硬感、颈部阳性体征、诊断性神经阻滞有效以及颈部影像学检查结果阳性是最主要的表现。结论:颈部僵硬感、颈部阳性体征、诊断性神经阻滞有效以及影像学阳性结果是颈源性头痛最主要的诊断依据。

Objective： To analyze the clinical symptoms,signs and imaging examination in patients with cervicogenic headache（CEH） and to discuss the diagnosis criteria of cervicogenic headache.Methods： 108 CHE patients who had hospitalized in our hospital from March 2007 to March 2009,were randomly and sequentially divided into A and B group.Use Logistic regression analysis to study the clinical condition such as age,sex,course of disease,VAS,position and character of headache,sense of neck rigidity,positive signs of neck and imaging examination of neck.Results： Sense of neck regidity,positive signs of neck,positive findings in imaging examination of neck and effective diagnostic nerve block are most important in conditions CEH patients.Conclusion： Sense of neck rigidity,positive signs of neck,positive findings in imaging examination of neck and effective diagnostic nerve block are main diagnositic criteria of CEH.
