择期胃癌手术取消常规留置胃管的效果

Effect of the gastric cancer patients without traditional nasogastric tube placement in the elective surgery

目的 探讨择期胃癌手术患者选择性留置胃管的安全性和可行性.方法 对2006年1月至2009年5月期间267例择期胃癌手术患者进行回顾性分析,其中置管组137例,未置管组130例,比较两组患者术后胃肠功能恢复时间、并发症、不适症状、再置胃管率等.结果 两组患者术后胃肠功能恢复的时间、并发症、再置管率,差异无统计学意义（P＞0.05） 置管组咽部疼痛的发生率和总并发症发生率明显高于未置管组,差异有统计学意义（P＜0.01）.结论 择期胃癌手术患者取消常规放置胃管在一定程度上促进了患者的康复,并缩短了住院时间,故择期胃癌手术不必要常规留置胃管.

Objective To investigate the safety and validity of the selective nasogastric tube placement in the gastric cancer patients undergoing elective operation by retrospective analysis. Methods A retrospective chart review of 267 patients operated electively for gastric cancer between 2006.1 and 2009.5 was performed at a single institution, tube group （n = 137 ）, no tube group （n = 130）. The time to recovering gastrointestinal function, nasogastric tube replacement, complications and discomfort were compared between the two groups.Results No significant differences in postoperative gastrointestinal function recovering time, nasogastric tube replacement rates and complications rates were observed between the two groups （ P 〉 0.05 ）, except that throat sere rates were significantly higher in the tube group（ P 〈 0. 01 ）. Conclusions Routine prophylactic nasogastric decompression is unnecessary after elective gastrectomy for gastric cancer patients.
